BNF has the URL, maybe copy from there. Also good to add VIAF. I sometimes creates new humans by just filling out the label and description and then only adding only one claim manually, VIAF=1234, and afterwards using User:Magnus Manske/ac2wd.js. Looks good. Now on any page that has VIAF, GND, maybe BNF there should be a link having label "AC2WD" in the left side menu in the section "Tools". If VIAF etc. has been added after page loading, one would need to reload the page to see the "AC2WD"-link. Q3301 - you see it at the left side?

In preferences there is a section gadgets, it has "DuplicateReferences: Adds a link to copy references and add them to other statements on the same item." - have you activated that?
